Thoughtfully designed homes for independent living
Achieve together has its own highly experienced Clinical and Quality Support Team who provide a comprehensive range of award winning e-learning, coaching and face to face training for all team members. These teams consist of specialist trainers including qualified Learning Disability Nurses, Psychologists, and Healthcare Facilitators. The specialist trainers support a range of people with autism spectrum conditions, behaviours that may challenge, mental health needs, and epilepsy. This ensures that team members have the skills and knowledge to support people effectively.

A safe and supportive place to call home
The Community Outreach Service supports people with a range of aspects and in varying degrees dependent on individual needs. People can choose the degree to which they are supported, and will be assisted in any area of their lives that they require which can include appointments, medication, social and leisure activities, self-care, budgeting, daily living skills, holidays and employment to name a few.
We are experienced with supporting people with personal budgets and direct payments and can provide all relevant support. We will help people recruit support team members from advertising to interviewing potential candidates as well as taking care of the paperwork.

The Community Outreach team are committed to helping people we support take part in community activities, have active social lives where possible and attend work placements and/or education placements. We ensure that each person takes part in meaningful activities that enrich their lives and build on new and existing skills including social and communication skills.
People will be supported in developing their life skills such as self-care, meal preparation, budgeting and domestic tasks in order to promote independent living.
