Your placement journey
1. Getting in touch
Contact us directly, or referrals can be made by family members, local council social workers, healthcare professionals, and other relevant authorities.
2. Finding suitable options
Our Assessment and Placement Managers (APMs) gather information to identify the most suitable homes and support options. We work closely with social and healthcare professionals to understand formal diagnoses, relevant medical history, and specific care needs.
3. Visiting potential homes
The individual looking to move in will be invited to visit potential homes, where our experienced Home Managers will discuss needs and preferences in a welcoming and supportive environment.
4. Personalised assessment
Our Home Managers conduct comprehensive assessments in collaboration with social and healthcare workers, ensuring we fully understand the individual's unique requirements and preferences for long-term well-being.
5. Confirming your support package
We work with social workers to formalise the chosen home and support plan, providing a transparent offer and clear support package agreement, detailing all aspects of care and residency.
6. Moving in
Our teams ensure a smooth and carefully planned transition into your new home, with support to help the person moving in settle and thrive from day one.
