The Wheel of Engagement (WoE) is Achieve together’s person-centred framework – coproduced with people we support, families, and support teams. It ensures we focus on what matters most in someone’s life, from identity and wellbeing to relationships, aspirations and independence.

A complete view of what matters in someone’s life

The Wheel of Engagement is our central person-centred planning tool. It helps us make sure nothing important is missed when supporting someone.

It guides our thinking, planning, and daily practice – linking directly to initial assessments, support plans and keyworker reports.

We see the person – not the label

At the outer edge of the Wheel, we focus on:

  • Personal identity – Who the person is, what shapes them, their story
  • Strengths and talents – What they love, what they’re great at
  • Choices and decisions – What they choose for themselves

We begin here, because this is where support with dignity truly starts.

Health, hope and feeling safe

Next, we explore the person’s wellbeing across every area:

  • Physical health – Health promotion, prevention and planning
  • Mental health – Emotional wellbeing, therapies, support strategies
  • Spiritual wellbeing – Faith, beliefs, a sense of meaning and connection
  • Personal safety – Feeling and being safe at home, online and in the community

Friends, family, identity and belonging

We help people stay connected to the people and relationships that matter – and explore new ones where appropriate.

This includes:

  • Family life and communication
  • Friendships and social connection
  • Sexual identity and healthy relationships
  • Positive rapport with professionals and support team members

Everyday life that feels meaningful

The next layer of the Wheel focuses on the person’s daily life, routines and sense of contribution, including:

  • Daily living skills – Maximising independence
  • Employment and education – Purpose and productivity
  • Inclusive communication – Expression and understanding
  • Hobbies and Interests – Enjoying and exploring
  • Sensory Experiences – Understanding sensory preferences and challenges
  • Being part of a community – Being a good neighbour and active citizen
  • Managing emotions and self-regulation – Coping well with life’s challenges

 

Independence at the heart of it all

At the very centre of the Wheel is the ultimate goal: autonomy and independence.

For some, this means reducing paid support and building life skills. For others – particularly people with profound and multiple disabilities or age-related conditions – it may mean maintaining skills and control for as long as possible.

We set SMART goals (Specific, Measurable, Achievable, Realistic, Time-bound) to help people lead lives that reflect their values and voice.

We embed the Wheel into everyday practice through dedicated training, delivered across all teams. This ensures:

  • Every support plan is person-centred
  • Every team member knows what good support looks like
  • The focus stays on the whole person – not just their diagnosis
