Finding the right support and home for yourself or a loved one can be a complex process. At Achieve together, we're here to guide you through every step, ensuring a smooth and person-centred experience.
Our expert placement service
Our dedicated Assessments and Placements Team works tirelessly to match individuals with the support and living environment that best meets their unique needs. With over 420 homes across England and Wales, we have the capacity to provide specialised support for adults with learning disabilities, autistic people, and people with behaviour which challenge.
Proven success
In 2023-2024, our team supported over 200 people in finding the right support at the right time and moving into our existing and newly opened homes. Our successful partnerships with numerous local authorities are paving the way for collaboration with new authorities and future growth, allowing us to help even more people.

- Swift response: We respond to the majority of enquiries within 24 hours
- Thorough assessment: For qualified enquiries, we complete assessments within five working days
- Quick turnaround: We aim to make offers of support within 24 hours of assessment completion
- Effective tracking: Our Customer Relationship Management system helps us manage over 2,600 qualified enquiries received this year

Your placement journey
1. Getting in touch
Contact us directly, or referrals can be made by family members, local council social workers, healthcare professionals, and other relevant authorities.
2. Finding suitable options
Our Assessment and Placement Managers (APMs) gather information to identify the most suitable homes and support options. We work closely with social and healthcare professionals to understand formal diagnoses, relevant medical history, and specific care needs.
3. Visiting potential homes
You're invited to visit potential homes, where our experienced Home Managers will discuss your needs and preferences in a welcoming and supportive environment.
4. Personalised assessment
Our Home Managers conduct comprehensive assessments in collaboration with social and healthcare workers, ensuring we fully understand your unique requirements and preferences for long-term well-being.
5. Confirming your support package
We work with social workers to formalise your chosen home and support plan, providing a transparent offer and clear support package agreement, detailing all aspects of care and residency.
6. Moving in
Our teams ensure a smooth and carefully planned transition into your new home, with support to help you settle in and thrive from day one.
