Thoughtfully designed for independent living

This warm and welcoming supported living home in Sapley supports adults with autism spectrum conditions, moderate to severe learning difficulties, mental health needs, complex needs including behaviours that may challenge, restricted mobility, hearing impairments, emotional needs and epilepsy.

People are supported using a person-centred planning approach, where support plans focus on outcomes and are tailored toward individual needs and ambitions. People are encouraged to be as independent as possible whilst participating in a wide range of activities in and around their community. The team is available 24 hours a day, including a waking night team.

What we offer

A safe and supportive place

The home comprises of a five-bedroom house and a two-bedroom annexe. The annexe provides a real sense of independence and has two en suite bedrooms with an open-plan kitchen/living room. The house is spacious and has three en suite bedrooms, with the remaining two sharing a family-size bathroom.

There are two living rooms and a kitchen diner that has been decorated by people at the home to meet their individual needs. One living room has been converted into a sensory room. There is a large patio and garden area with a trampoline and a hot tub, which is shared with the annexe.

This home is connected to an additional Achieve together home next door, and people living at both homes often get together.

The home is situated out of the town centre but only a short distance from all local amenities, including a bowling alley, gym, cinema and supermarket.

Lifestyle

People living here enjoy active lives and are supported to take part in a range of activities both within their home and the wider community in order to increase social and communication skills. Leisure activities that people take part in include day centre activities, walks in the surrounding areas, gardening, spa days, bowling, sailing, swimming, gym sessions, and trips to the cinema. People are also supported to participate in many educational, voluntary and work placements of their choosing, and many people are currently building confidence by volunteering, which greatly increases independence.

The home is generally a quiet environment with moments of busyness.

Everybody at the home interacts really well, and firm friendships have been created.

People are supported on any day trips and holidays they wish to go on.
