Every year, our Heroes awards shine a spotlight on the people and teams who go above and beyond to make a difference – from team members and managers to people we support.

What are the Heroes awards?

The Heroes awards are more than just a ceremony. They’re a celebration of real people, real stories and real impact – co-produced with people we support.

Anyone can make a nomination – whether you’re a person supported, family member, friend, team member or professional – and every nomination helps us celebrate the everyday moments of brilliance that happen across our organisation.

Award categories

Being brave

This award celebrates a team member or person we support who has shown courage by stepping outside their comfort zone to try something new, overcome challenges or achieve something they never thought possible.

This could be through building greater independence, developing new skills, overcoming personal barriers or making a positive difference to their own life or the lives of others.

We want to hear about people who embrace new opportunities, face challenges with determination and inspire those around them through their resilience and bravery.

This award may also recognise someone who has joined Achieve together within the last year and is new to the health and social care sector. Tell us how they have already demonstrated our values through exceptional courage and commitment while making a positive impact.

Having fun

This award celebrates a team member, team or person we support who brings energy, positivity and enjoyment into everyday life.

We want to hear about individuals or teams who create opportunities for people to celebrate achievements, build confidence and make lasting memories. They inspire others through their enthusiasm, encourage participation and help create a vibrant, inclusive and enjoyable home or workplace where everyone feels valued and involved.

Making things happen

This award recognises a team member, team or person we support who has transformed ideas into meaningful action, creating lasting positive outcomes for people we support.

We are looking for examples of ambition, creativity and determination that have led to real improvements, whether through developing new ways of working, solving challenges or making opportunities happen.

This award may also recognise someone who has made an exceptional contribution to their local community through volunteering, paid employment, fundraising, conservation work or being a great neighbour. Tell us how they have turned passion into action and inspired others along the way.

Valuing everyone

This award recognises a team member, team or person we support who champions inclusion, celebrates individuality and empowers others to achieve their full potential.

This could be someone who’s demonstrated commitment to co-producing projects and/or advocating for people supported. How have they made change happen?

Or it could be someone who has wholly embraced person-centred care, promoted independence, choice and control, supporting people to live happy, healthy and meaningful lives.

 

Wellbeing and health promotion award

This award celebrates those who promote the emotional, physical, and psychological health of people we support.

We seek excellent examples of how teams or team members or people we support have supported people to enjoy good health, overcome health inequalities and gain greater control over their own health and lives.

How has this enabled them to manage their health and wellbeing so they can maximise their independence, choice and control?

Inspirational person award

This award recognises a person we support or team member whose determination, resilience or achievements have inspired others.

They may have overcome significant personal challenges, reached an extraordinary goal or demonstrate exceptional strength in the face of adversity.

Share their story and tell us how their journey has motivated others, made a lasting impact and embodied the values of Achieve together.

Home / service of the year

This award celebrates a home or service where people we support and team members work together to create a welcoming, inclusive and thriving community.

We want to hear about outstanding teamwork, meaningful relationships and opportunities that enrich people's lives. This could include community involvement, fundraising, events, partnerships or innovative ways of bringing people together.

Tell us how the home or service creates an environment where everyone feels valued, connected and supported to achieve their full potential.

Community hub team/team member of the year

This award recognises a Community Hub team or team member that has made an outstanding contribution to supporting our homes and services, team members and the wider organisation. We are looking for teams / team members who work collaboratively, provide exceptional support and expertise, and consistently go above and beyond to help others succeed.

Share examples of how they have demonstrated teamwork, built strong relationships, solved problems and embodied our values while helping others achieve great outcomes.

How to nominate

  1. Read the awards categories below and determine which one relates best to the nominee 
  2. Fill in the information in the form about yourself (the nominator) and the nominee 
  3. Send any supporting documents to help the judges see why the nominee is so deserving of the award 
  4. If you would like to nominate someone for more than one category, please start a separate submission form per category

Please note, if the nominee is shortlisted, they will be required to attend their region's in-person awards ceremony: 

North – Tuesday 3rd November 2026, Stoke on Trent

Wales & West – Tuesday 10th November 2026, Newport

South & East – Tuesday 24th November 2026, Uckfield

Heroes awards 2025: A celebration to remember

For the first time, we took the celebrations across the country, with three regional events full of energy and excitement.
Each one brought inspiring, courageous and joyful stories to the forefront - all reflecting the values that truly unite us.
