Our Community Outreach Service in Reading supports adults with mental health needs. Support is provided throughout the Reading area, in the person’s family home, own home and in the community. People are supported to live as independently as possible, and can choose the level of support they require from 1 to 24 hours a day.

Achieve together has its own highly experienced Clinical and Quality Support Team who provide a comprehensive range of award winning e-learning, coaching and face to face training for all team members. These teams consist of specialist trainers including qualified Learning Disability Nurses, Psychologists, and Healthcare Facilitators. The specialist trainers support a range of people with autism spectrum conditions, behaviours that may challenge, mental health needs, and epilepsy. This ensures that team members have the skills and knowledge to support people effectively.
