About this home

Thoughtfully designed homes for independent living

Hazel House is a specialist residential home providing tailored support for adults with learning disabilities, autism and other neurodivergent conditions. People may present with complex communication difficulties, sensory processing issues, behaviours which may challenge or secondary mental health conditions. Located close to Grantham town centre, Hazel House offers a calm, supportive environment designed to meet a diverse range of needs.

Hazel House delivers support with dignity, respect, and an unwavering commitment to person-centred support. Every person supported is seen and supported as the person they are, with bespoke support packages shaped around their unique strengths, preferences, and goals.

What we offer

A safe and supportive place to call home

Hazel House can support up to four adults, with individual bedrooms and shared bathrooms. The living space is a good size with a lounge, separate dining room and large kitchen. There is a utility area, and a small office located at the back of the property. There is a large gated driveway to the front of the property and a large garden to the rear of the house for the people supported to access. The skilled team members provide the best support while promoting as much independence as possible for the people supported.

Lifestyle

The home and garden area are designed to meet the individual’s complex needs, offering individual space as well as access to communal areas for recreation and skill development.

People supported receive support with their daily activities and decision making. The skilled team members offer activities such as gardening, swimming, walks, days out and learning essential life skills such as menu planning, cooking and shopping, all aimed at supporting independence and boosting self-confidence.
