Digital Transformation
Using technology to enhance lives
At Achieve together, digital transformation isn’t about replacing hard working people – it's about empowering them.
We’re using technology to improve the lives of people we support, streamline how our teams work and make our services even safer, smarter and more person-centred.
A clear digital vision
Our digital transformation is rooted in simple belief:
Technology should support people, not systems.
That’s why we’re building a connected, accessible and sustainable digital infrastructure that aligns with our core values – and keeps people at the centre of everything we do.
We’re proud to support the NHS Digital Maturity Assessment Framework, which champions digital adoption across health and social care.
Smart systems, strong support
Here’s how our digital journey is making a difference:
We’ve introduced a new Customer Relationship Management (CRM) system that helps us respond to referrals faster and more effectively – making sure people receive the support they need, when they need it.
Our new asset and maintenance platform allows teams to log repairs and manage safety checks instantly – keeping homes safe, comfortable and running smoothly.
Our ‘Every Visit Counts’ approach helps us gather regular feedback from the people we support, their families and our team members – helping us to spot opportunities for improvement in the moment.
Our digital tool PASS+PORT™ helps us personalise support plans and carry out quality reviews more efficiently – improving quality of life and reducing duplication.
Our partnership with RADAR gives us a universal system to track incidents, share learning and act on risks quickly – reducing reliance on paper and helping us stay ahead.
Supporting people, not replacing them
We know that great support is built on relationships – not just systems. That’s why our approach to digital transformation is always human-first.
We use digital tools to:
- Give team members more time for direct support.
- Empower people we support through accessible platforms and feedback.
- Build confidence and digital skills across homes and services.
- Reduce admin and duplication so teams can focus on what matters.
What's next
We’re continuing to invest in tools, systems and training that make life better for people we support - and easier for the teams who work alongside them.
Our digital roadmap includes:
- More mobile-friendly platforms to support teams wherever they are.
- Digital care records to improve information sharing, care coordination and decision-making.
- Enhanced team rostering tools for smarter shift planning and greater team wellbeing.
- Real-time dashboards to improve oversight and drive faster action.
- Assistive and inclusive technology to promote independence and accessibility.
- Stronger alignment with national digital best practice in health and social care.
