Learning disabilities: Support that’s personal and outcome focused

At Achieve together, we support people with learning disabilities to live happy, healthy and meaningful lives – on their own terms. No two people are the same, so our support never is either. Each person will have their own aspirations, strengths and talents – we will focus on these to support each person to live their best life.

What is a learning disability?

A learning disability is a lifelong condition that affects how a person understands and processes information, learns new skills, and manages everyday life. It starts before adulthood and can be mild, moderate, severe, or profound.

Every person’s experience is unique — and so is our support.

Everyone is different – and that’s where we begin

Some people’s learning disability is linked to a specific condition like Down Syndrome or Prader-Willi Syndrome. Others may also be autistic, have ADHD, mental health conditions, or physical disabilities.

Whatever someone’s situation, we focus on their abilities, ambitions, and the life they want to live.

Support that’s skilled, respectful, and shaped by the individual

We offer the right support, in the right way, at the right time. That might be in a supported living home, a residential home, or through outreach services.

Our teams are trained and guided by expert clinicians, including:

  • Nurse-led health promotion team
  • Positive behaviour support (PBS) team
  • Mental health specialists
  • Forensic support experts

We build trusted relationships and treat every person with respect, tailoring support to their needs – always guided by evidence and best practice.

It looks like choice, progress and connection

Support at Achieve together means helping people build skills, make decisions, and enjoy life. We work towards increasing independence wherever possible, and reducing paid support over time –  always at the person’s pace.
