Moving from children’s services to adult support can be a big step for young people and their families. At Achieve together, we make the journey smoother, safer and more supported.

A big change – with the right support

Transitioning to adulthood can feel exciting, overwhelming, and anxiety provoking all at once. We work closely with the young person, their family, and local authorities to create a personalised support plan that puts their aspirations, needs and relationships at the centre.

We help maintain what’s working – such as communication tools, trusted routines, and friendships – while opening new doors to education, supported employment, independence and personal growth.

Planning together, around what matters most

We use our Wheel of Engagement Person-Centred Framework to explore what’s important to each young person – their voice, their routines, their identity. Every support plan is tailored to the person, considering:

  • Their EHCP (Education, Health & Care Plan)
  • Inclusive communication tools and sensory needs
  • Existing health or therapy support
  • Goals around independence, friendships and identity
  • Options for education or supported employment

From school days to supported living – your next chapter starts here

We provide a range of adult support settings – including supported living and residential homes – a new place for the young person to call home. Our teams are trained to make each transition as smooth, stress-free and confidence-boosting as possible.

We also work with local education providers, social services and funding authorities to make sure the transition is smooth — with no gaps in communication, support and at a pace the young person is comfortable with.

Exploring what’s next, together

Whether it’s college, training or a supported job, we help young people build confidence and skills for the future – often supported through EHCP funding.

We also offer safe, respectful support around gender identity, sexuality and personal relationships – helping each young person feel fully seen and supported.
