Continuity matters – especially during change
We make sure key support doesn’t fall through the cracks when services change. That includes:
- Carrying over EHCP (Education, Health & Care Plan) support
- Maintaining communication tools and therapy input
- Involving you and your family in every step of planning
- Ensuring the new team understands what matters to you
Residential, supported living or community support – it’s your choice
We’ll help you explore the best support setting, based on what works for you:
- Comfortable, adapted homes in friendly communities
- Supported living to build independence
- Outreach or community-based support
- A place where you can grow, connect and belong

Explore learning, work and doing what you love
We’ll support you to find purpose and pride in your day. That might include:
- Continuing education or life skills learning
- Supported internships or volunteering
- Finding new hobbies or social groups
- Getting involved in your community

Growing up includes exploring who you are
We support young people to express who they are – safely and respectfully. This includes:
- Talking about gender or sexual identity
- Navigating friendships and relationships
- Building confidence and emotional resilience
- Staying safe while growing in independence
