Nurse-led support that transforms care

Skilled nurses. Safer support. Stronger outcomes.

Our award-winning internal nursing team brings specialist healthcare insight, clinical guidance and hands-on support – ensuring people thrive with safe, person-led support in their own communities. All clinical support, protocols, training and competency assessment is based on a clear evidence base.

Support shaped by clinical excellence

At Achieve together, we support people with complex physical, postural, neurological, ageing and mental health needs. Our in-house nurse-led team plays a vital role in guiding our support through:

  • Evidence-based practice
  • Advanced clinical insight
  • Specialist training and competency assessment
  • Delegated Healthcare Activities (e.g. enteral feeding, non-invasive ventilation, complex medication management)
  • Strong Clinical Governance, minimising risk with robust auditing systems, clear evidence-based healthcare policies and protocols

We work alongside local health services to ensure each person gets the right care — focusing on health promotion, prevention and proactive planning.

2) Warm, dignified portrait

Reducing risk. Promoting dignity. Supporting wellbeing.

Our nursing team ensures that support teams have the confidence, clarity and competence to deliver complex care safely.

They provide:

  • Clinical governance oversight
  • Guidance on risk assessment and reduction
  • Advocacy and safeguarding support, especially in cases of health inequality
  • Support with critical health decisions

Together, we’re building safer, more equitable lives for the people we support.

Here to enhance, not replace

We work alongside NHS and local health professionals – not instead of them. Our nursing team enhances support by:

  • Translating clinical guidance into daily support practice
  • Providing advice and signposting to managers
  • Stepping in during complex care or decision-making
  • Ensuring continuity when services are stretched
  • Support teams to implement the PMLD Core & Essential Service Standards

This joined-up approach helps people live well in their own homes, while easing pressure on NHS and community teams.

Compassion when it matters most

When teams are supporting someone at the end of life, our nurses step in to provide:

  • Guidance on end-of-life planning and care
  • Emotional support for the circle of support
  • Practical oversight to ensure a dignified, peaceful death
  • Debriefing and wellbeing support for teams afterward

We believe every person deserves a good death – and every team deserves support in helping make that happen.

3) Quiet, respectful image of a support worker holding someone’s hand

Led by nurses. Guided by purpose.

Our nursing team has been recognised for its compassionate, forward-thinking approach. Their work helps drive:

  • Policy and practice improvements
  • Team member development and training
  • A stronger, safer experience for the people we support

Every day, their presence helps us raise standards — and keep people at the heart of everything we do.
