Meet Jay: the importance of family connection

Jay has profound and multiple learning disabilities (PMLD) and started his journey with Achieve together at his first home, Meesons Lodge in Essex. As his family live in East Sussex and have busy lives, travelling to visit him was sometimes difficult. Jay’s mother contacted Achieve together to explore the possibility of him moving closer, but there were no available homes that suited his needs at the time.

When an opportunity arose, Achieve together arranged a temporary move to Lynfords while arrangements with his previous home were completed. During this time, a request was made to Jay’s funders for a permanent placement, which was approved within a few months.

First Steps

Lynfords was well suited to Jay’s moving and handling needs, helping him feel comfortable and well supported in his home. To make his move as smooth and reassuring as possible, team members from Meesons Lodge, who already knew Jay, worked closely with the Lynfords team. Jay settled in well and enjoyed regular visits from his parents.

Once Lynfords was confirmed as his permanent home, his support plan was reviewed, and referrals were made to local healthcare teams. The team also arranged updated equipment tailored to Jay’s physical needs and reassured his family that they would always receive a warm welcome at his home.

Ongoing support

Following a review of Jay’s support and healthcare needs, his dietary requirements have been identified as an area where further support is needed. His GP and a dietitian are working with him to complete a formal assessment and ensure he receives the right support.

His home is fully adapted to make everyday life easier, with accessible hallways, doorways and space to move around comfortably. Tracking hoists are available in every room and in communal areas, while adapted kitchen work surfaces provide easy access and a clear view. These adaptations help Jay enjoy everyday activities, such as watching cooking and baking sessions.

Jay also enjoys playing his clackers and listening to the sounds they make. The team joins in with this activity to help him feel heard and understood, while offering other sensory experiences, including lights and sounds. Alongside this, the team encourages Jay to build his independence and enjoy activities locally and further afield.

Jay’s progress

Jay’s move to Lynfords has brought him closer to his family and helped strengthen their relationships. His mother visits regularly, while his father spends time with him at home and takes him out for something to eat. Jay has also enjoyed several day trips to visit his family, and his parents have taken him to a light festival. Being closer gives them more opportunities to spend quality time together and stay connected.

Jay enjoys attending local events, including festival nights and parades, where he can watch the shows, see the costumes and take in the atmosphere. He also loves bowling, trips to the cinema and relaxing in front of the TV at home, especially watching Cars. His joyful, outgoing personality shines through in the connections he makes with others.

Looking to the future

The future looks full of possibilities for Jay as he continues to enjoy regular, unrestricted quality time with his family. The team will support him to build his independence, explore more of his community and discover new activities that reflect his likes and interests, updating his support plans along the way.

These opportunities will give Jay new ways to express his cheerful, social personality. He will also be going on holiday with his peers, and the team is exploring adapted venues that meet his needs so he can enjoy his time away.
