Meet Jacob: new adventures and a life shared
Jacob moved from his family home to a supported living home in Ryde in 2019. At first, Jacob needed support with his mental health, particularly anxiety, which affected his confidence in going out and enjoying activities away from home.
Communication was initially a challenge, as Jacob found it difficult to express how he was feeling and what he wanted. He was also keen to become more independent and develop everyday life skills. The team focused on helping Jacob understand his feelings, find positive ways to manage his anxiety and communicate his wishes, giving him greater choice in his everyday life.

First Steps
From the beginning, the team involved Jacob in planning his support and setting goals he wanted to pursue. Team members listened without judgement and worked alongside him to explore ways to communicate his feelings and wishes. Clear, respectful conversations gave him opportunities to talk through choices, possible outcomes and ways to stay safe.
Support was consistent and followed Jacob’s pace, with a focus on understanding his emotions and finding practical ways to manage situations he found difficult. Skateboarding became part of his routine, offering an enjoyable way to stay active and express himself. Together, these approaches provided tools he could use in everyday life while working towards greater independence.

Ongoing Support
After moving from supported living into his own home to increase his independence, Jacob continued to receive outreach support. He now visits the supported living home in Ryde each week. Team members offer a listening ear and practical guidance with decisions, appointments and everyday responsibilities. He chooses what to discuss and how support is provided.
These conversations give Jacob space to explore his feelings, consider his options and ask for guidance when he wants it. This ongoing approach fits around his work, home life and interests.

Jacob’s Progress
By 2021, Jacob had secured paid work and felt ready for a new chapter. He met his partner in 2022, and they decided to move in together before marrying in September 2025.
Today, Jacob lives independently with his wife and works for a well-known fast-food establishment, while she works as a make-up consultant. He enjoys skateboarding, spending time with his wife and exploring activities that offer new experiences and opportunities to develop his social skills. As his confidence has grown, Jacob has become more able to recognise when guidance would be useful and choose how it fits into the life he has built.

Looking to the Future
Jacob is looking forward to continuing a happy, fulfilling life with his wife. He wants to remain in work and make lasting memories with the people who matter most to him.
His next exciting plan is a holiday to Disneyland, offering the chance to explore somewhere new and spend quality time with his wife. Looking further ahead, Jacob and his wife hope to start a family when the time feels right for them. Outreach support will remain available to Jacob, offering encouragement or a chance to talk things through. With clear aspirations and valued relationships around him, he feels ready to embrace the next chapter.
