About this support setting

Thoughtfully designed for independent living

Domiciliary Care Staffordshire provide bespoke care for adults with learning disabilities, autism and mental health issues. The experienced and skilled support team support people to live as independently as possible, providing the right level of support when it is needed most. In order to provide a life outside long-stay hospital settings, we provide each person the support they need to live a life of their choosing.

Support we offer ranges from live-in day and night time support to community and social support for a few hours per day. Achieve together are also in a position to offer a wide range of accommodation options for each individual. This could be in a residential or supported living setting or the person’s own home. We have the ability, through our housing partners to provide solutions to complex housing problems across Stafford, Cheshire and Stoke-on-Trent.

What we offer

A safe and supportive place

The team work on the principle that the outcomes of forensic support and supervision are essentially the same as in the wider population, that is to diminish pain and suffering and to function healthily and freely in society.

We work collaboratively with NHS trust services such as teams from mental health,  learning disability, substance misuse, primary care and social care.

Our care is always person-centred in order to improve the experience and engagement of people we support. We do this by using evidenced-based tools in order to provide care that is specific to the varying complex needs of the person using the service.

In addition, each person is given the motivation they need to live a safe, pro-social lifestyle.

Lifestyle

We can introduce people we support to new groups, hobbies and socialising possibilities, and we can also help them learn key socialising and relationship skills too.

Achieve together adopt a strength-based approach, where we look to create opportunities for meaningful activities, interests, occupation and often employment to develop fulfilling lives.

We also focus on improving how each person functions in their own right by enhancing his or her capabilities to attain goals and confidence.

Finally, we view care planning and delivery through a trauma lens by adopting the Trauma-Informed Approach to care and support.
