Partnerships with Achieve together
Partnering for better lives
At Achieve together, we know that meaningful partnerships help people thrive.
Whether you’re commissioning services, driving innovation in the sector, or providing goods and expertise – we work closely with you to deliver high quality, person-centred support for people who draw on social care, requiring high levels of skilled support.
Commissioning our services
We partner with local authorities, Integrated Care Boards (ICBs) and health and social care commissioners to deliver tailored, high-quality support across England and Wales.
What you can expect:
- A wide range of community-based support models, from supported living to more tailored options that reflect individual preferences and needs.
- Strong track record of supporting people who have experienced lengthy hospital stays, multiple breakdowns in support and services and who may have experienced significant trauma in their lives.
- Specialism in supporting people with learning disabilities, autistic people and behaviours that may challenge.
- Homes and services co-designed to support positive outcomes and independence.
- Strong clinical and governance oversight, including input from Positive Behaviour Support (PBS) and a Multidisciplinary Team (MDT).
- At Achieve together, we’re proud to provide safe, person-centred support that helps people live happy, healthy and meaningful lives.
- A trusted partner to reduce pressure on acute and inpatient settings.
- Transparency in our approaches and commitment to ensuring value for money for commissioning authorities.
- A healthy interest in how AI and Assistive Technology can support advances for the benefit of people who draw on social care.

We don’t just deliver support – we help shape how the sector evolves.
