What kind of care do I want?

The options available for support are wide ranging and differ depending on individual circumstances and choice. Here at Achieve together we pride ourselves on offering person centred support plans and services that are co-produced with the individual, their families and social workers or community teams.

Whilst being person centred and focusing on what the person wants and needs is the key to everything we do, there are different models of support that we can provide this through.

Support in any of our services is provided by our expert teams who are trained to meet the needs of the individuals we support, offering comprehensive support packages for people with Autism and Learning Disabilities, people with Profound and Multiple Learning Disabilities, people with Mental Health needs, people with Forensic Support, people with Acquired Brain Injuries and more.

Where a person needs support with any personal care, we can provide this either at registered care home, or if in supported living or home care through our care services.

Residential care:

Residential support offers peace of mind, safety and security with accommodation that is managed by us and there is always support in that home day and night.

With residential support you don’t need to worry about paying for your rent or your utility bills as these homes belong to Achieve together so the residential fee that pays for you to live in that home covers all of this, along with your support.

You will share your home with other people who we are supporting, and we will make sure that as part of any move that you and the other people we support have time to get to meet each other and to check that you are all happy with sharing your home.

If you do choose residential support you will live at a registered care home which will be inspected by the regulator, either CQC in England or CIW in Wales, who will publish a report about what they find. You can find all our current reports online.

Supported living:

Like our residential care our supported living homes focus on providing support that is about what you want and need. Support can range from help with day-to-day tasks, being active in the community, to making friends and gaining employment.

However, unlike registered care the level of support that is provided can be less, and while we could have 24-hour support in place, not all of our supported living services have team members available on site 24 hours a day.

If you do choose supported living you may be sharing with others, or you may live alone, but in both cases you will have your own tenancy agreement which means that you have your own contract with the Housing Association that own or rent that building that you live in. This is a legally binding agreement which means that you are living in your own home, and you can choose who provides your support. You will get housing benefit to pay your rent, and you will need to budget to pay your bills and pay for your food and other things that you need to live your life, including activities and outings. The support team can help you to budget if you need them to.

Living in a supported living setting we can still support you with any personal care that you need through our domiciliary care registrations and any personal care we provide will be inspected by the regulator, either CQC in England or CIW in Wales, who will publish a report about what they find. Supported living can be a great stepping stone for building confidence for those who want to live even more independently in the future.

Outreach/Home care/Domiciliary:

Outreach provides support for day-to-day tasks within your own home. This allows you to maintain independence with the reassurance of professional support when you need it. Support options can include help washing and dressing, attending appointments, financial budgeting and leisure pursuits and employment opportunities. This is a great option for people who are transitioning into independent living for the first time from a more structured support service”.

At Achieve together our experienced and award-winning teams specialise in supporting people with wide ranging support needs.
