Meet Teresa: taking the next step towards independence

Teresa moved to Winston Lodge in October 2024. Before she arrived, the team received information about the support she needed in daily life. A Positive Behaviour Support (PBS) plan provided a starting point for understanding her needs and guiding the team’s approach.

However, the earlier assessments did not fully reflect Teresa’s strengths, skills or independence. As the team got to know Teresa as an individual, they gained a better understanding of her abilities, goals and what was important to her.

First steps

When Teresa first moved to Winston Lodge, the team helped her feel safe, valued and at home. She needed time and support to adjust to her new surroundings and get to know the people around her. By building trusting relationships, the team developed a better understanding of her preferences, what she could do independently and where she needed their support.

Teresa enjoyed helping to prepare lunch and took pride in contributing to life at home. With encouragement, she began volunteering at a local charity shop, meeting people and building meaningful connections. As her confidence grew, she also began going out independently, taking her mobile phone with her for reassurance and safety.

Ongoing support

During her time at Winston Lodge, Teresa experienced periods of distress and needed support with behaviours that may challenge. The team worked with her GP, psychology professionals and an Achieve together Positive Behaviour Support (PBS) specialist to understand her needs and adapt her support.

Teresa also shared her wish to live more independently. Together, the team and professionals reviewed her support plan and communication needs, exploring how they could support her to work towards this long-term goal.

Teresa’s progress

In September 2026, Teresa moved to a supported living home, taking an important step towards the greater independence she wanted.

Teresa now has more opportunities to make each day her own, choosing the activities she wants to do and planning the meals she would like throughout the week. From shopping for groceries to cooking her meals, she puts those choices into practice.

She also does her laundry and keeps her personal space tidy. These everyday tasks are an important part of living more independently, giving Teresa more say in how she spends her time and looks after her home. Her support remains tailored to her needs and aspirations.

Looking to the future

Teresa is looking forward to growing in confidence and continuing to volunteer. She wants to keep learning practical skills, take on meaningful roles and responsibilities, and maintain the positive relationships that matter to her.

Her support will continue to adapt to her needs and what matters to her. Teresa’s choices will guide her next steps as she works towards her goals at her own pace.
