Meet Stephen: moving from Residential to Supported Living

Steven moved to Achieve together from a hospital setting and started his amazing journey at Cwm Hyfrd Residential Home. In 2014 Steven was able to transition into our Supported Living home in Newport, firstly living on the ground floor and later moving to a room on the first floor. There was a big difference when Steven moved from residential care to supported living and this did take some time for Steven to adjust, but with support, encouragement and guidance from team members Steven has flourished.

First Steps

When Steven first arrived he expressed an interest in building his independence. He wanted to set himself goals, the first of which was to administer his own medication. Team members sat down with Steven and asked him how this would look to him. It was agreed that a phased approach would be sensible with Steven administering his medication with team members present until he gained more confidence. Once confident, Steven was able to do this himself and team members just check the balance of his medication and record it.

Next on Steven’s list was independently completing his weekly shop. Again, this was approached with small steps, Steven started with smaller shops and gradually built this up, he now goes by himself each week and is really proud of his progress.

Ongoing Support

Steven was continuing to thrive and recently made a great step forwards in his independence. Steven had to move out of the main house whilst some refurbishments were completed on his flat. He enjoyed his ‘holiday home’, an outside flat at the home, so much that he wanted to stay there. The team supported him to complete a transfer request to the local authority, which has been granted and the move is now officially complete.

Steven’s Progress

Steven’s progress has been astonishing, his confidence and happiness has grown and he’s so proud of what he’s been able to achieve. Steven’s story is a fantastic example of co-production in action. Every step has been informed and developed to meet Steven’s goals, with the team building small step change processes to achieve the end goal – a real team effort. Everything that has been achieved so far has laid the foundations for Steven to continue to be
more independent in the future.

Looking to the future

Steven has settled well into his new flat and has expressed an interest in making it into his ‘man cave’ where he can relax after a busy day. With support from the team and his appointee, Steven will be purchasing items in the coming months to make it his own.

Steven’s next goal is to work on managing his finances by himself, something we are confident he can achieve.
