Thoughtfully designed homes for independent living
This supported living home supports adults with learning disabilities, autism spectrum conditions, mental health needs and Asperger’s who benefit from living in their own space with a bespoke support package to suit their needs.
The experienced support team are focused on outcomes and support people living here to develop life skills. People receive support and encouragement with becoming active participants in their wider community, and the team liaise with family members, with consent from those they support, regarding people’s individual care and support.

What we offer
A safe and supportive place to call home
This home is a modern property with six individual self-contained flats, including ground-floor flats. Each flat has its own bathroom, kitchen lounge/diner and can be individually fitted out to meet specific needs.
The home is ideally located near the local swimming pool and leisure centres, where people are supported to explore and enjoy the facilities on offer, including gyms, saunas and access to sports parks.

Lifestyle
This home is a quiet and peaceful home located on a quiet road.
Everybody here is independent and requires minimal support. People get on well with each other, and although people are busy independently during the day, they enjoy catching up over a meal together in the evenings.
The home is neighbour to another Achieve together home, and the two communities regularly come together in the rear garden to enjoy activities and celebrations. People are supported in taking part in any leisure activities, with others such as swimming, bowling, trips to the day centres, art classes, visiting local restaurants and trips to the cinema.
People are supported in building on their independence skills, such as budgeting, self-care and meal preparation.
We are very proud of three people who have moved onto more independent lifestyles and settings after having spent most of their lives in residential care. We continue to support these people via our outreach support service.
