Understanding the transition process

As young people approach adulthood, typically around the age of 18, their support moves from children’s services to adult social care. This change involves new systems, different funding structures and fresh opportunities. Starting conversations early (we recommend by the age of 14) can help set future goals and ease the transition for everyone involved.

Most people in supported living have a tenancy agreement in their own name, giving them more choice and control over where they live. While care and support are funded by the Local Authority, people use their own benefits to pay for rent and bills – just like anyone else living independently.

 Our supported living options provide:

Residential homes

  • Support in building life skills and achieving personal goals.
