Thoughtfully designed for independent living
Cwm Hyfryd is a 24-hour residential home supporting adults with autism spectrum conditions, moderate learning disabilities, complex needs, including behaviours that may challenge, restricted mobility and sight impairment.
Individuals at Cwm Hyfryd are supported to develop individualised person-centred plans and positive behaviour support plans that promote increased choice, control over their lives and independence. Our experienced team works in partnership with families, carers, advocates and public service professionals to ensure that the support provided is proactive and appropriate. Our Total Communication approach enables staff to implement individualised communication strategies for service users with communication difficulties.

What we offer
A safe and supportive place
Cwm Hyfryd is a warm and homely property situated in Pontllanfraith. It is a large detached bungalow with five bedrooms with wheelchair accessibility. There is a communal living room, dining room, fully equipped kitchen, sensory room, and conservatory. There is also a large garden which hosts various leisure activities, especially during the summer.
The home also has access to a house vehicle, so day trips to surrounding areas are easily possible.

Activities such as Touch Trust, equine therapy, bowling, disco, arts & crafts, gardening, walks, sensory sessions, active support, day trips, theatre visits, and café visits are also supported by the team at Cwm Hyfryd.
