Learn about Rob’s successful transition to his new home

Since a young age, Rob has had several admissions into hospital, in total of 7 years (his last admission was 3 years). Understandably, Rob was anxious about moving into the community and had taken some time over the past 18 months to view potential opportunities as he had previously not felt settled.

After a well-planned assessment and several visits to his proposed new home in the Wirral area, Rob felt that this home was the best match for himself. Not only had he gained trust with our team, he also met other people we support, and spent time at his new home to ensure any anxieties and concerns were eased before his move.

Rob was given the support he needed prior to moving. It’s safe to say that he is thoroughly enjoying his new home and getting to know everyone! Rob’s previous placement provider, health professionals, and family have all fed back about how well the team have supported Rob with his transition and settling in, and how happy Rob is in his new home.
