Welcome to our newly refurbished supported living home in Sutton, designed to support adults with learning disabilities, emotional needs, forensic, and mental health challenges. Situated in a spacious semi-detached property, our home features six en-suite bedrooms, providing comfort and privacy for people supported.

At this home, our dedicated team prioritises building strong connections with people supported, working closely with family members and professionals to develop tailored care plans focused on both short and long-term outcomes. We actively seek opportunities for people supported to engage in enriching activities and employment, striving to deliver the highest quality of care and support.

Embracing a tailored and person-centered approach, we empower people supported to reach their full potential. Through positive risk-taking and community reintegration initiatives, we aim to enhance their independence. This philosophy has resulted in successful transitions to supported living for many people supported, with a notable reduction in support levels.

Achieve together provides bespoke support packages tailored to each person supported’s unique needs, guiding them in discovering meaningful activities within the local and wider community. This includes access to vocational activities, college, and opportunities for both voluntary and paid work.

Every support package is meticulously crafted and subject to ongoing reviews to ensure the least restrictive option is maintained. Our support team undergoes specialised training specific to the needs of the person they support, in addition to mandatory training, showcasing our commitment to their ongoing development. We highly value our workforce and offer ample opportunities for their professional growth and advancement.

Our collaborative efforts extend to working with health and social care colleagues, including Mental Health Nurses, Learning Disability Nurses, Occupational Therapists, Positive Behaviour Support Specialists, and Social Workers, all aimed at achieving the best outcomes for people supported.

The team are available 24 hours a day to support in all aspects of daily life, from cooking, laundry, support finding employment, understanding bills and growing further independence, along with other day-to-day activities. We can also accommodate supported sleeping, and there will always be a waking night team member on hand.

Our spacious semi-detached house, features six cozy bedrooms, each equipped with en-suite facilities. In addition to their private spaces, people supported can also enjoy socialising in our inviting communal areas, including the spacious kitchen, lounge, and dining room.

Our property boasts a lovely garden, providing people supported with a peaceful outdoor retreat. For those seeking a quieter space, we offer a small, tranquil room that people supported may find especially beneficial for relaxation or personal reflection.

Lifestyle

People are supported to take part in activities that increase independence and improve communication and social skills. This includes support in daily life, such as household tasks and meal preparation. People can also choose to seek out and explore work experience and paid work placements in order to maximise their quality of life and learn new skills.

Emphasising a vibrant communal atmosphere, we encourage socialising in various shared spaces, including the kitchen, lounge, dining area, and conservatory. The flexibility of large spaces allows for conversion into a games room, sensory room, or quiet room, adapting to the unique needs of those we support.

Nestled in the lively area of Sutton, the town centre is conveniently just ten minutes away, offering easy access to local shops, restaurants, and leisure activities, including the cinema. With excellent transport links to central London and surrounding areas, our location provides a gateway to a diverse range of opportunities and experiences.

Support extends to assisting individuals in finding paid or voluntary work and actively participating in the broader community, which may include joining social groups such as football, gym sessions, cinema outings, bowling, and BBQs. This home caters to those who are more independent, capable of verbal communication and managing most day-to-day activities but benefit from assistance and encouragement in areas like managing finances, medication, household tasks, and community access.
