Welcome to our newly refurbished supported living home in Sutton, designed to support adults with learning disabilities, emotional needs, forensic, and mental health challenges. Situated in a spacious semi-detached property, our home features six en-suite bedrooms, providing comfort and privacy for people supported.
At this home, our dedicated team prioritises building strong connections with people supported, working closely with family members and professionals to develop tailored care plans focused on both short and long-term outcomes. We actively seek opportunities for people supported to engage in enriching activities and employment, striving to deliver the highest quality of care and support.
Embracing a tailored and person-centered approach, we empower people supported to reach their full potential. Through positive risk-taking and community reintegration initiatives, we aim to enhance their independence. This philosophy has resulted in successful transitions to supported living for many people supported, with a notable reduction in support levels.
Achieve together provides bespoke support packages tailored to each person supported’s unique needs, guiding them in discovering meaningful activities within the local and wider community. This includes access to vocational activities, college, and opportunities for both voluntary and paid work.
Every support package is meticulously crafted and subject to ongoing reviews to ensure the least restrictive option is maintained. Our support team undergoes specialised training specific to the needs of the person they support, in addition to mandatory training, showcasing our commitment to their ongoing development. We highly value our workforce and offer ample opportunities for their professional growth and advancement.
Our collaborative efforts extend to working with health and social care colleagues, including Mental Health Nurses, Learning Disability Nurses, Occupational Therapists, Positive Behaviour Support Specialists, and Social Workers, all aimed at achieving the best outcomes for people supported.
The team are available 24 hours a day to support in all aspects of daily life, from cooking, laundry, support finding employment, understanding bills and growing further independence, along with other day-to-day activities. We can also accommodate supported sleeping, and there will always be a waking night team member on hand.
