Ty Coedwig is a supported living service for adults aged 18+ with learning disabilities, physical disabilities, behaviours that may challenge, autism spectrum conditions and mental health needs.
We will follow an active, person-centred approach which enables people to pro-actively manage their mental health needs, make informed choices and achieve their planned outcomes.
People will be supported to develop individualised Health Action Plans which include access to a range of healthcare professionals.
We will work closely with community teams to ensure that tenants continue to access key services and appointments. This close working relationship enables us to focus on increased independence and recovery for people who have mental health needs.
The service helps to build resilience in people through the promotion of positive risk-taking and through equipping people with practical, transferable skills. The service aims to reduce dependence on staff and provide the necessary skills for the individuals supported to thrive independently both inside and outside of the home so that they might then be able to move on to greater independence in the future.Download Brochure
