This supported living home, in Sittingbourne supports adults with learning disabilities, with a focus on developing further independent skills.
The team are able to support people from the age of 17 as part of a transitional model of support. The focus of the team is to promote independence, employment opportunities and person-centred transition plans for independent living.
We’re very proud of some who have moved onto their own flats in the community, and we continue to provide outreach support to them. They have made fantastic progress and achieved some very positive personal outcomes in relation to independence and quality of life.
